If you need to change the information provided:

Please disable your API key used in submitting the form and create a new API key to resubmit a new form with the correct information.

If you want to use multiple accounts:

Simply submit using the form again

Note: All trade transactions will be mapped to only one account, so even if you submit the same account multiple times, only one of them will be randomly assigned as the owner of the trade. i.e only one of the accounts will receive the reward.
